The Form 759 is used by localities to record an assessment of individual or fiduciary income tax. This does not apply to localities who use an automated system for tracking assessments. The Form 759 is used internally by localities and is not submitted to TAX. Detailed instructions are also provided to complete the Form 759. (Effective January 2009)
Instructions
Two versions of the form are provided. Each version has two pages that must be completed.
PRINTABLE:
Print blank copies of the "Form 759 Printable" version if you wish to have paper copies that are filled in by hand.
FILLABLE:
Use the "Form 759 Fillable" version if you wish to complete the form on your PC. The fillable document may be saved on your PC and printed if you wish a paper copy.
